Which diagnosis best fits duration, episodicity, and exclusions?
A
Stereotypic movement disorder
Repetitive, seemingly driven, and apparently purposeless motor behaviour (e.g. hand shaking/waving, body rocking, head banging, self-biting, hitting own body)
Presence of obsessions (recurrent/persistent intrusive thoughts/urges/images + attempts to suppress or neutralise), compulsions (repetitive behaviours/mental acts driven by rigid rules aimed at reducing distress/feared event), or both
AA. Repetitive, seemingly driven, and apparently purposeless motor behaviour (e.g. hand shaking/waving, body rocking, head banging, self-biting, hitting own body) B. Interferes with social, academic, or other activities and may result in self-injury C. Onset in the early developmental period
BA. Presence of obsessions (recurrent/persistent intrusive thoughts/urges/images + attempts to suppress or neutralise), compulsions (repetitive behaviours/mental acts driven by rigid rules aimed at reducing distress/feared event), or both B. Obsessions/compulsions are time-consuming (>1 hr/day) or cause clinically significant distress/impairment C. Not attributable to physiological effects of a substance or another medical condition
Common specifiers
AWith self-injurious behaviour; Without self-injurious behaviour; Associated with a known medical or genetic condition; Mild
BWith good or fair insight; With poor insight; With absent insight / delusional beliefs; Tic-related
Differential flags
ATic disorders (sudden, rapid, brief; premonitory urge; suppressible with effort; wax and wane); OCD (ego-dystonic; anxiety-driven compulsions; not rhythmic); ASD (may coexist - code separately if ID criterion met independently)
BGAD (worry is ego-syntonic, future-focused, realistic - OCD obsessions are ego-dystonic, intrusive, irrational); OCPD (ego-syntonic perfectionism without obsessions/compulsions); MDD (rumination is mood-congruent, not intrusive; no compulsions)
